Your next step

Start with a conversation.

Every family’s situation is different. Talna’s team can listen to your concerns, explain hospice care, and help you understand the next steps in Murrieta and surrounding communities.

Preparing for a care conversation

You do not need to have every answer before calling. It can help to discuss your loved one’s goals, current concerns, living situation, and the people involved in their care.

  • What matters most to your loved one right now?
  • Which symptoms or daily tasks are becoming difficult?
  • Who is available to help, and what support do caregivers need?
  • What questions do you have about hospice, coverage, or visits?

Discuss medical details directly with the care team. Do not send medical records or sensitive health information through website forms.

What happens next?

  1. 01

    Talk with Talna

Share your questions and the best way to reach you.

  2. 02

    Review care needs

The team discusses evaluation, physician certification, and coverage.

  3. 03

    Plan together

If hospice is appropriate and elected, the team creates an individualized plan with the patient and family.

Caring for the caregiver

Rest, meals, and time to talk matter for caregivers too. Ask the team about practical guidance, community resources, and short-term inpatient respite when hospice benefit criteria are met.

Hospice visits support the care plan but do not replace a full-time caregiver. Talk openly about the help needed at home.

Support through grief

Grief can begin before a death and may affect each family member differently. Ask about bereavement resources for adults and caregivers, and appropriate referrals for children and teens.

Planning and reliable information

Talk with your healthcare provider about advance directives and who should speak for you if you cannot make medical decisions. A qualified professional can help with legal questions.
